REBAR: 296-155-680: Each employee on the face of formwork or reinforcing steel shall be protected from falling 6 feet (1.8m) or more to lower levels by personal fall arrest systems
Different than the 10 foot rule applied to all other fall protection.
Positioning devices shall be secured to an anchorage capable of supporting at least twice the potential impact load of an employee's fall or 3,000 pounds.
Training: WAC 800-16035: Employees Must be in writing (or on computer) names, dates, subjects. Does NOT have to be one form per employee.

The employer shall develop and implement a written fall protection work plan including each area of the work place where the employees are assigned and where fall hazards of 10 feet or more exist.

Exception: General contractor specific contract language requiring specific fall protection above DOSH standards. DOSH/OSHA is MINIMUM Standards!
Positioning devices (rebar assembly) shall be rigged such that an employee cannot free fall more than 2 feet.
